    Emerging Roles for Diabetes Educators

    Ann Constance, MA RD CDE - 2012 AADE Diabetes Educator of the Year

    Objectives:

    • Describe how Patient Centered Medical Homes can enhance management and support for people with diabetes or other chronic conditions.
    • Identify ways that diabetes educators may be imbedded in PCMH
    • Demonstrate importance of community based support and discuss roles for diabetes educators in providing or linking to support.
    • Discuss policy and environmental changes that support healthful behaviors and functions diabetes educators may fill.
    • Compose one or two actions plans to implement based on something discussed during workshop.
